Client Forms & Informative Documents

There are many forms involved when caring for a client. We have compiled a list of the most used forms for quick access, in the event you need to submit an initial intake form, lost a page of your previous packet, or just want to have contracts on hand for obtaining our services at a future date.

If you wish to submit an updated form to our office, you can email it to main@thecomfortabilityhomecare.com, text it to (719) 357-7485, or submit it via "Contact Us" section below, which allows for upload of a document. 

Please include clients first name, last name, and client ID (along with your information if you are not the client). If you have any questions or concerns please call us anytime.
